Description

You'll conduct comprehensive patient assessments, provide skilled nursing care, and coordinate hospice services after hours. You'll work a rotating schedule with one week on-call (overnight shifts and weekends) followed by a full week off.

Requirements

• Bachelor's or Associate's degree in nursing or related field
• Active RN license in good standing in Indiana
• Active CPR certification
• 2-5 years experience in healthcare management, hospice, or palliative care preferred
• 1 year experience with Medicare Hospice Certification and CHAP guidelines preferred

Responsibilities

• Complete initial and comprehensive patient assessments to determine hospice needs
• Provide professional nursing care and administer medications/treatments as prescribed
• Develop and revise care plans in collaboration with patient, family, and interdisciplinary team
• Coordinate after-hours patient care and support Case Managers
• Maintain accurate clinical documentation and communicate with physicians
