💼 About This Role

You'll perform home health visits for adult and geriatric patients in Carbon County. Your core impact is providing rehabilitative care that helps clients remain at home. Using a state-of-the-art touch pad tablet, you'll document care and collaborate with a multi-disciplinary team.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform evaluations for speech, language, communication, or swallowing difficulties.
• Develop and revise treatment plans based on evaluations and physician orders.
• Accurately document observations and interventions using a touch pad tablet.

📋 Requirements

• Current license as a Speech Language Pathologist in Pennsylvania.
• Certificate of clinical competence from ASHA or equivalent educational requirements.
• Solid computer skills; prior experience with electronic medical records preferred.
