- Maintain ongoing caseload using evidence-based approaches to promote engagement.
- Conduct telephonic and SMS outreach for timely follow-up.
- Collaborate with nurse care manager and behavioral health manager on delegated tasks.
- Assist members in securing community supports and scheduling appointments.
๐ Requirements
- 1+ years of professional experience in case management or healthcare call center.
- Bilingual in English and Spanish.
- Strong cultural fluency and ability to build trust with diverse populations.
- Reliable HIPAA-compliant workspace with stable internet.
