  • Collaborate with nurse care manager and behavioral health manager on delegated tasks.
  • Assist members in securing community supports and scheduling appointments.

  • 1+ years of professional experience in case management or healthcare call center.
  • Bilingual in English and Spanish.
  • Strong cultural fluency and ability to build trust with diverse populations.
  • Reliable HIPAA-compliant workspace with stable internet.
