Description

As a Jefferson Health at Home by BAYADA Nurse, you'll provide 1:1 skilled nursing care to patients in their homes, including wound care, medication administration, and patient education. You'll conduct wound care consultations, perform client assessments, and monitor patients' conditions, helping them transition from hospital to home with comfort and independence.

Requirements

• Active and current RN license in Pennsylvania
• WOCN certification required
• At least one year of nursing experience
• Prior home care experience strongly preferred
• Ability to travel and strong assessment/communication skills

Responsibilities

• Deliver 1:1 skilled nursing care including medication administration, wound care, and treatments
• Conduct wound care consultations and provide oversight
• Perform client assessments and provide education to patients and families
• Monitor patients' condition and support safe home life
• Travel to patient cases as assigned
