💼 About This Role

You'll perform home orientations and assessments for patients in Connecticut's structured family caregiving program. Your work ensures caregivers provide excellent service to loved ones while supporting patient independence. Ongoing training and interdisciplinary collaboration are key parts of this role.

🎯 What You'll Do

• Conduct home orientations and sign paperwork
• Perform initial assessments for new patients
• Complete periodic supervisory visits for on-care cases
• Train caregivers and patients as required per case

📋 Requirements

• 2+ years experience with frail or elderly disabled adults
• Ability to travel throughout New Haven area and surrounding cities
• Valid driver's license with insured vehicle
• Proficient in Microsoft Office (Word, Excel, Outlook)

✨ Nice to Have

• Previous field experience in medical or case management
• Salesforce experience
• Working knowledge of ADL/IADL assessments
