You'll provide personal care under an RN Case Manager's supervision, assisting with bathing, dressing, and mobility. Your work directly helps maintain our under-10% hospital readmission rate. We've been named a 2024 Top Workplace and #1 home health agency in San Francisco.

🎯 What You'll Do

• Provide personal care including baths, oral hygiene, and bed linen changes
• Assist patients with dressing, undressing, and toileting activities
• Perform range of motion exercises and other therapy services as ordered
• Report patient condition and significant changes to assigned nurse

📋 Requirements

• Valid CHHA CA Certification
• Current BLS/CPR Certification
• Valid CA Driver's License
