Description

As a home care Occupational Therapist, you will perform home visits to evaluate patients' functional status, develop individualized intervention plans, and educate patients and families on rehabilitative care, enabling clients to receive care at home.

Requirements

• Current DE Occupational Therapist license
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of patients' functional status
• Develop individualized intervention plans to achieve favorable outcomes
• Evaluate home environment for hazards and identify equipment needs
• Educate patients and family members on rehabilitative care
