Description

You will provide frontline care to underserved and unhoused populations in the San Francisco Bay Area, working with a multidisciplinary team to deliver acute, chronic, preventative, and palliative care in outdoor and community-based settings. This role combines hands-on clinical care with AI-driven tools to make a meaningful impact beyond traditional healthcare.

Requirements

  • MD or DO degree
  • Active, unrestricted California Physician Surgeon license
  • Board certification or eligibility (preferred specialties: Family Medicine, Internal Medicine, Emergency Medicine, Palliative Care, Geriatrics, Addiction Medicine/Pain Management)
  • DEA registration or eligibility to obtain
  • National Provider Identifier (NPI) number
  • Basic Life Support (BLS) certification
  • 2+ years of clinical experience providing care to underserved populations (homeless, SMI/SUD, transition from incarceration, complex medical needs)
  • Experience in street medicine or community-based settings
  • Strong familiarity with Bay Area medical, social, and community resources for underserved populations
  • Comfortable providing care in outdoor, field-based settings under dynamic conditions
  • Strong interpersonal and communication skills
  • Ability to travel up to 75% of work time; valid California Driver's License and access to reliable, insured vehicle

Responsibilities

  • Provide frontline care to patients experiencing homelessness, behavioral health challenges, serious mental illness, substance use disorders, and complex chronic conditions.
  • Manage diverse patient panels delivering acute, chronic, preventative, and palliative care directly in the field.
  • Conduct patient assessments, gather histories, perform physical exams, order and interpret diagnostics, and prescribe treatment.
  • Collaborate with a multidisciplinary care team including nurses, social workers, and outreach staff.
  • Design and implement care plans supporting patient stability and improved health outcomes.
  • Provide clinical mentorship to other care team members.
  • Engage in palliative or specialty-specific care planning for patients with serious or life-limiting illnesses.
  • Document all encounters accurately for continuity of care.
