Description

You will plan, organize, and direct hospice care for patients and families, conducting comprehensive assessments, developing care plans, and coordinating with the interdisciplinary team to ensure compassionate end-of-life support.

Requirements

• Registered Nurse (RN) license
• Experience in hospice or community health nursing
• Strong assessment and care planning skills
• Ability to work collaboratively in an interdisciplinary team
• Knowledge of palliative care principles

Responsibilities

• Complete initial and ongoing comprehensive assessments
• Develop and revise individualized care plans
• Administer medications and treatments per physician orders
• Coordinate with interdisciplinary team and community resources
• Maintain accurate clinical documentation and communicate patient changes
