Description

You will assess the psychosocial status of patients and families facing terminal illness, collaborating with the interdisciplinary team to provide compassionate, holistic end-of-life care in the patient's residence.

Requirements

• Master's degree in Social Work (MSW)
• Ability to perform psychosocial assessments

Responsibilities

• Assess psychosocial status of patients and families related to terminal illness
• Communicate findings to registered nurse and interdisciplinary group
• Conduct assessments in patient's identified residence
• Assist when residence is unsafe and alternative plan required
