Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes, performing evaluations and developing treatment plans for speech, language, communication, or swallowing difficulties. You'll be part of a multi-disciplinary team, using a touch pad tablet for documentation.

Requirements

• Current Speech Language Pathologist license in New Jersey
• Certificate of clinical competence in speech from ASHA or equivalent educational requirements
• Solid computer skills; prior experience with electronic medical records preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluation and physician’s orders
• Educate and instruct patients and family members on rehabilitative care activities
• Document observations, interventions, and evaluations using an electronic touch pad tablet
