Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes. You'll perform evaluations, develop intervention plans, and educate patients and families to promote health, safety, and independent living.

Requirements

• Current Maryland Occupational Therapist license
• Minimum one year of recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Perform thorough evaluations of patients’ functional status (muscle function, endurance, visual coordination, communication, ADL/IADL)
• Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans
• Evaluate home environment for hazards and identify equipment needs and environmental adaptations
• Educate and instruct patients, family members, or representatives in rehabilitative care and activities
• Accurately document patient care observations, interventions, and evaluations using an electronic medical record system
