Description

You will provide home health speech therapy services to adult and geriatric patients in Davidson County, NC. As an integral member of a multi-disciplinary team, you will conduct evaluations, develop treatment plans, and educate patients and families to help clients remain safely at home.

Requirements

• Current Speech Language Pathologist license in North Carolina
• Certificate of Clinical Competence in Speech from ASHA
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations for speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluations and physician orders
• Educate patients and family members on rehabilitative care and safety
• Document observations and interventions using a touch pad tablet
