Description

As a Client Services Manager, you will coordinate care for Assistive Care clients, manage staffing and scheduling, communicate with clients and caregivers, and process referrals to help adults and seniors live safely at home.

Responsibilities

• Manage a caseload of Assistive Care clients, ensuring schedules are filled and services delivered reliably
• Coordinate day-to-day staffing and scheduling for personal care and companion services
• Communicate with clients, families, and caregivers to resolve issues and maintain quality
• Monitor and respond to open shifts, cancellations, and coverage gaps
• Receive and process referrals from hospitals, discharge planners, payors, and community partners
