💼 About This Role

You'll coordinate care for high-complexity patients, working mainly in the field to provide chronic care coordination and support. You'll directly engage with patients through home visits, phone calls, and community interactions, assessing needs and helping set health goals. This role focuses on increasing access to preventative care and reducing emergency room visits.

🎯 What You'll Do

• Conduct home visits and community outreach to engage patients
• Assess patient needs and coordinate care with interdisciplinary teams
• Help patients set health goals and develop self-management plans
• Document patient interactions and maintain accurate records in EMR

📋 Requirements

• 2 years of experience as Community Health Worker, Peer Support, or similar
• High school diploma or GED required
• Valid driver's license and auto liability insurance
• Experience coordinating care for chronic or behavioral health conditions

✨ Nice to Have

• Multi-lingual capabilities
• Prior home care or Enhanced Care Management experience
• Community Health Worker certification
