💼 About This Role

You'll provide skilled nursing and rehabilitative care to adult and senior clients on a per visit basis, working as part of a multi-disciplinary health care team. You'll help clients remain at home with one-on-one care, supported by 24/7 clinical backup.

• Monitor clients' conditions and perform assessments
• Follow up on and execute doctors' orders
• Document observations, interventions, and evaluations
• Coordinate case management and travel to clients

📋 Requirements

• Active Colorado RN nursing license in good standing
• Graduation from an accredited nursing program
• Current or recent Medicare Home Health and OASIS documentation experience
