💼 About This Role

You'll provide hands-on therapeutic interventions for patients in their homes, working under a PT Case Manager. This role focuses on building core clinical skills in a growing home health agency recognized as a top workplace. You'll contribute to low hospital readmission rates through quality care.

🎯 What You'll Do

• Provide therapeutic interventions like gait training and manual therapy.
• Conduct patient evaluations and implement care plans under supervision.
• Document patient care accurately within 24-48 hours.
• Educate patients and caregivers on mobility and safety techniques.

📋 Requirements

• Current unrestricted California PT License.
• Current BLS/CPR Certification.
• Graduate of an accredited Physical Therapy program.
• Valid CA Driver’s License with clean driving record.

• 1 year of PT experience preferred; new grads considered.
• Prior home health experience highly desirable.
