Description

You will provide rehabilitative care and skilled nursing to clients in their homes, performing evaluations, developing individualized intervention plans, assessing home environments, and educating patients and families to promote health and independent living.

Requirements

• Current Maryland Occupational Therapist license
• Graduation from an accredited occupational therapy program
• Minimum one year of recent work experience as an Occupational Therapist
• Ability to work independently and manage time effectively
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of functional status and ADL/IADL performance
• Develop individualized intervention plans and revise care plans as needed
• Identify equipment needs and environmental adaptations for enhanced independence
• Educate patients and families on rehabilitative care and document interventions
