Description

You will provide home health visits to adult and geriatric patients, delivering compassionate care and helping them maintain comfort and independence in their homes.

Requirements

• One year prior clinical experience as a licensed RN
• Previous home health care experience preferred

Responsibilities

• Follow designated care plan per patient needs
• Make home visits in designated geographic territories
• Administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes to Clinical Manager
• Execute and document doctors' orders
