Description

You will provide home health speech therapy to adult and geriatric patients in senior living communities, performing evaluations, developing treatment plans, and educating patients and families to support independent living.

Requirements

• Current Speech Language Pathologist license in VA
• Certificate of Clinical Competence in Speech from ASHA or equivalent educational/work experience
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Perform home visits for speech, language, communication, or swallowing evaluations and therapy
• Develop and revise treatment plans based on evaluations and physician orders
• Educate and instruct patients and family members in rehabilitative care and independent living activities
• Document patient care management and services using a touch pad tablet
• Participate in multidisciplinary care plan meetings
