Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes in Lancaster County. You'll perform diagnostic tests, develop care plans, and educate clients to promote independent living.

Requirements

• Current Pennsylvania Physical Therapist license
• Minimum one year recent work experience as a Physical Therapist
• Graduation from an APTA or CAAHEP approved program
• Strong interpersonal and organizational skills
• Ability to work independently and manage time effectively

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, balance, strength, etc.)
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and family members on rehabilitative care and safety
• Document observations and interventions using a touch pad tablet
