Description

You will work with clients in their homes as part of a multi-disciplinary care team, crafting and implementing treatment plans to help them reach their goals. You'll perform diagnostic tests, develop individualized care plans, and educate clients and families. This role offers flexibility and a supportive environment.

Requirements

• Current Physical Therapist license in Hawaii
• Graduation from an APTA or CAAHEP accredited program
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Perform diagnostic tests and measurements (mobility, range of motion, balance, etc.)
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Implement interventions for wound prevention and repair using evidence-based dressings
• Educate clients and families on rehabilitative care to promote health and independence
• Document observations, interventions, and evaluations using electronic medical records
