💼 About This Role

You'll conduct independent nursing visits focusing on prevention and chronic disease management for senior patients. You'll coordinate with providers and community resources to deliver high-quality, whole-person care. This role is part of One Medical's Senior Health Care Team, offering the opportunity to impact patient outcomes in a primary care setting.

🎯 What You'll Do

• Conduct independent nursing visits focusing on prevention, medication reconciliation, and chronic disease follow-up.
• Support patients in managing their care, navigating the healthcare system, and understanding their conditions.
• Coordinate with primary care providers and health navigators on follow-up care for high-risk patients.
• Identify and act on open care gaps including cancer screenings, vaccines, and cognitive assessments.

📋 Requirements

• Licensed RN in California required.
• 2+ years of nursing experience in a hospital, post-acute facility, or ambulatory setting.
• Experience in care management, value-based care, or population health preferred.
• Ability to work 8am-5pm Pacific Time and commute within the greater Bay Area.

• Bachelor of Science in Nursing or MSN preferred.
• Experience with motivational interviewing and patient-centered communication.
