💼 About This Role

You'll provide direct patient care as part of an interdisciplinary team, serving families in their homes. Your work will help build trust and connect patients to critical health and social services. You'll combine clinical skills with advocacy and navigation support to address social drivers of health.

🎯 What You'll Do

• Perform vitals, specimen collection, and assist with exams in homes
• Administer medications and vaccinations under clinician direction
• Complete accurate EMR documentation for each patient visit
• Coordinate referrals to external social care providers

📋 Requirements

• Completed Medical Assistant program
• 1-3 years of relevant experience
• 1 year phlebotomy experience
• Active BLS certification

• Spanish language skills
