Description

You will conduct in-home assessments and deliver personalized home modification recommendations to help older adults and individuals with disabilities live safely and independently. You'll collaborate with a clinical operations team and maintain documentation through an electronic system.

Requirements

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Reliable transportation and valid driver's license

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized home modification recommendations
• Educate clients and caregivers on fall prevention and safe mobility
• Engage clients and families in collaborative care planning
• Maintain accurate and timely documentation
