You'll serve as an attending physician for patients in skilled nursing facilities, collaborating with NPs/PAs to manage daily care. You'll provide high-quality primary care and participate in quality initiatives like Chronic Care Management. 🎯 What You'll Do

• Serve as attending physician for patients in assigned skilled nursing facilities.
• Collaborate with NPs/PAs to manage daily patient care and provide clinical supervision.
• Diagnose and treat acute and chronic conditions in a patient-centered primary care model.
• Participate in Chronic Care Management, Transitional Care, and Remote Patient Monitoring initiatives.

📋 Requirements

• Active U.S. medical license as an MD/DO.
• Board-certified or board-eligible in an appropriate specialty such as Internal Medicine or Family Medicine.
• Ability to perform essential job functions with or without reasonable accommodation.
