Description

You will lead field staff in delivering exceptional client service and quality clinical care, visit clients in their homes, supervise field clinicians, and assist with in-home client assessments and care planning. Your work ensures clients come first and caregivers have the support they need.

Requirements

• Current New Hampshire RN license in good standing
• Minimum two years recent clinical field experience in Medicare Certified Home Health
• Experience with clinical oversight including OASIS and ICD-9 Coding
• Strong assessment and organizational skills
• Solid communication and PC skills

Responsibilities

• Visit clients in their homes and supervise field clinicians
• Assist with in-home client assessments and care planning
• Support internal case management and evaluation of field staff
• Provide clinical insight and support for the team
• Work with both patients and office staff
