💼 About This Role

You'll conduct home health admissions and assessments for a growing home health agency serving patients across the Bay Area. Your work will directly reduce hospital readmission rates by providing high-quality, compassionate care in patients' homes.

🎯 What You'll Do

• Conduct Start of Care (SOC) admissions and OASIS assessments
• Administer IV medications (antibiotics and diuretics) safely
• Collaborate with physicians and interdisciplinary team
• Document patient observations and interventions accurately

📋 Requirements

• CA RN license unrestricted
• BLS/CPR certification
• Valid CA Driver's License
• Experience with Start of Care (SOC) - OASIS

• Experience with high acuity patients
• Previous IV medication infusion proficiency
