💼 About This Role

You'll join a collaborative interdisciplinary team to provide primary care to seniors in a value-based care model. You'll manage a smaller caseload, enabling you to form long-term relationships and focus on holistic patient outcomes.

🎯 What You'll Do

• Provide continuous medical coverage and perform comprehensive in-person assessments
• Collaborate with Physicians and IDT to develop participant care plans
• Assist in maintaining participant care in skilled nursing facilities and residences
• Review outcomes and utilization data to establish best practices and policy improvements

📋 Requirements

• Graduation from an accredited school of nursing
• Certificate as a Nurse Practitioner or Physician Assistant with prescriptive authority
• Minimum of two (2) years of experience working with frail or elderly populations
• Able and willing to take part in rotating night and weekend call
