In this role, you will coordinate home care services, manage client caseloads, and support field staff to ensure high-quality, reliable care for adults and pediatrics. You will process referrals, manage scheduling, and partner with clinical teams to grow the caseload.

• Proficiency with scheduling software and billing systems

Responsibilities

• Manage a caseload of clients ensuring schedules are filled and services delivered reliably
• Coordinate day-to-day staffing and scheduling for personal care and companion services
• Communicate regularly with clients, families, and caregivers to understand needs and resolve issues
• Receive and process referrals from hospitals, discharge planners, and community partners
• Track billable hours and caseload growth to contribute to office productivity
