💼 About This Role

You'll provide home health physical therapy to help patients regain independence and live safely in their homes. As a case manager, you'll develop care plans and coordinate with physicians and PTAs. This role offers flexible scheduling and a supportive team culture.

🎯 What You'll Do

• Perform home evaluations and develop care plans
• Supervise Physical Therapy Assistants (PTAs)
• Communicate and coordinate with patients' physicians
• Evaluate and report patients' progress to MD

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
