Description
You will directly outreach and engage with individuals living with serious mental illness, substance use disorder, experiencing homelessness, or with high medical needs, building trust and relationships to connect them with whole-person care. You'll serve as a Lead Case Manager, working remotely with 2-3 community on-site visits per month.
Requirements
- Bilingual in Spanish and English
- Passionate about serving underserved populations
- Experience with serious mental illness, substance use disorder, or homelessness preferred
- Ability to work collaboratively in an interdisciplinary team
