💼 About This Role

You'll provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes as part of a multi-disciplinary team. You'll develop individualized care plans and educate clients and families to promote independent living. This role offers scheduling flexibility with PRN, part-time, or full-time options.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility and strength.
• Implement wound prevention and repair interventions.
• Develop and implement individualized care plans including therapeutic exercises.

📋 Requirements

• Current VA Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the APTA or CAAHEP.
