Description

You will be an integral member of a multi-disciplinary health care team, providing rehabilitative care and skilled nursing to clients in their homes. Perform evaluations, develop treatment plans, educate patients and families, and document care using a touch pad tablet.

Requirements

• Current license as a Speech Language Pathologist in Maine
• Certificate of clinical competence in speech from ASHA or equivalent educational/experience requirements
• Solid computer skills; prior experience with EMR preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluation and physician orders
• Educate and instruct patients and family members in rehabilitative care
• Document observations and interventions using electronic medical records
