Description

You will conduct in-home assessments and deliver personalized home modification recommendations for older adults and individuals with disabilities, helping them live safely and independently. This role involves travel to client homes, collaboration with a remote team, and using tech-first systems to document care.

Requirements

• Degree in Occupational Therapy from an accredited program
• 2+ years of clinical experience
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation

Responsibilities

• Complete onboarding and credentialing
• Conduct in-home assessments and functional evaluations
• Deliver personalized, evidence-based home modification recommendations
• Educate clients and caregivers on fall prevention and safe mobility
• Maintain accurate documentation through the electronic system
