💼 About This Role

You'll provide home health visits to adult and geriatric patients, evaluating functional status and developing individualized intervention plans. As part of a multi-disciplinary team, you'll enable patients to remain safely at home.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations of patients' functional status
• Assess cognitive, psychological and motor aspects of performance
• Educate patients and family members in rehabilitative care

📋 Requirements

• Current North Carolina Occupational Therapist license
• Minimum 1 year of recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
