💼 About This Role

You'll conduct Start of Care visits and deliver 1:1 skilled nursing in patients' homes. Your core impact is providing compassionate, high-quality care for clinically complex patients while maintaining flexible daytime schedules. 🎯 What You'll Do

• Perform 1-3 Start of Care visits per day in a small territory
• Deliver skilled nursing including medication administration and wound care
• Conduct client assessments and provide patient/family education
• Monitor patients' condition and adjust care plans as needed

📋 Requirements

• Current NJ nursing license in good standing
• At least one year of related experience, preferably in Home Health
• Comfortable utilizing an EMR; experience with HCHB a plus
• Experience with OASIS and completing Start of Care visits

• Previous home health experience
