💼 About This Role

You'll join Abby Care as a PASA Case Manager supporting Colorado's PASA program. You'll manage caseloads and coordinate care for families receiving Medicaid home health services. This role offers the chance to work with a tech-powered, family-first care platform.

🎯 What You'll Do

• Manage caseloads for PASA program participants
• Coordinate care and onboarding for new clients
• Ensure compliance with Medicaid home health regulations
• Document and report using Salesforce and Google Workspace

📋 Requirements

• 2-3 years of direct case manager experience in Colorado's PASA program
• Familiarity with CES/SLS Waivers and PASA services
• Experience in care coordination and caseload management
• Proficiency in Google Workspace or Salesforce for documentation

✨ Nice to Have

• Bachelor's degree in social work, nursing, or healthcare administration
• Experience with Community Connector, Homemaker Enhanced, or Respite services
