💼 About This Role

You'll perform home health visits as an integral member of a multi-disciplinary team. You'll evaluate functional status and develop individualized intervention plans to promote independent living. This role offers the opportunity to make a direct impact on clients' ability to remain at home.

🎯 What You'll Do

• Perform thorough evaluations of patients' functional status
• Develop individualized intervention plans to achieve favorable outcomes
• Educate and instruct patients and families in rehabilitative care
• Accurately document observations and evaluations using an electronic tablet

📋 Requirements

• Current New Jersey Occupational Therapist license
• Minimum one year of recent work experience as an Occupational Therapist
• Graduation from an accredited occupational therapy program
• Ability to work independently and manage time effectively
