💼 About This Role

You'll conduct Start of Care visits and deliver one-on-one skilled nursing care to complex patients in their homes. You'll expand your clinical skills while performing assessments, wound care, and medication administration across several New Jersey counties.

🎯 What You'll Do

• Perform 1-3 Start of Care visits per day
• Deliver 1:1 skilled nursing and supportive care
• Administer medication, wound care, and treatments
• Conduct client assessments and provide patient education

📋 Requirements

• Current NJ nursing license in good standing
• At least one year of related experience
• Comfortable using an EMR system
• Experience with OASIS and completing Start of Care visits

✨ Nice to Have

• Experience with HCHB EMR
• Home Health experience preferred
