💼 About This Role

You'll provide skilled nursing care to adult and geriatric clients in their homes, making home visits in Falmouth, Mashpee, Sandwich, and Bourne. You'll be an integral member of a multi-disciplinary health care team ensuring client well-being.

🎯 What You'll Do

• Make home visits to clients in designated territories.
• Administer medication, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes.
• Document care using electronic medical records on a tablet.

📋 Requirements

• Current LPN license in Massachusetts.
• One year clinical experience as an LPN.
• Graduation from an accredited nursing program.
