💼 About This Role

You'll provide in-home physical therapy to adult and geriatric patients in the St. Charles area. You'll help patients restore mobility, prevent falls, and live safely in their homes. This role offers flexible scheduling and a consistent patient census through our agency network.

🎯 What You'll Do

• Deliver in-home physical therapy to adult and geriatric patients
• Evaluate mobility, strength, and function for individualized goals
• Implement rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate documentation per Medicare and agency standards

📋 Requirements

• Active Physical Therapy license in good standing
• BLS CPR certification
• Reliable transportation for home visits
• Basic comfort with technology (email, texting, EMR)
