💼 About This Role

You'll create and execute care plans for home health patients in the Placerville area, working to prevent re-hospitalization through comprehensive assessments and coordination with an interdisciplinary team. You'll lead two nurses supporting your care plans and help set a new benchmark for patient outcomes.

🎯 What You'll Do

• Perform full patient and family assessments during initial visits.
• Develop and execute an advanced plan of care including rehabilitation.
• Collaborate with physicians and interdisciplinary clinical team members.
• Prepare clinical notes, orders, and charting in a compliant timeframe.

📋 Requirements

• Minimum 1 year of nursing experience required.
• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
