💼 About This Role

You'll provide hospice home visits to patients in Sullivan County, NH, following a designated care plan. You'll perform wound care, medication administration, and client assessments while ensuring accurate documentation using a touch pad tablet. Flexible scheduling and 1:1 care allow for great work/life balance.

🎯 What You'll Do

• Follow a designated care plan in accordance with patient's needs
• Perform home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, and procedures
• Accurately document observations, interventions, and evaluations

📋 Requirements

• Current Registered Nurse license in VT/NH
• Minimum one year of recent clinical medical/surgical experience
• Graduation from an accredited nursing program
• Ability to travel to cases as assigned
