You'll perform Start of Care (SOC) admissions and follow-up visits for a leading home health agency in the Bay Area. Your work directly impacts patient outcomes, helping maintain our under-10% hospital readmission rate.

🎯 What You'll Do

• Conduct Start of Care (SOC) admissions and OASIS assessments
• Make home visits and perform wound care, treatments, and procedures
• Initiate and administer IV medications safely per protocols
• Collaborate with physicians and interdisciplinary team for patient care

📋 Requirements

• Current CA RN license
• Current BLS/CPR certification
• Valid CA driver's license
• Experience with OASIS assessments highly desired

• Experience with high acuity patients
• IV medication infusion experience
• Case management fundamentals
