Requirements

• Active Registered Nurse (RN) license
• Minimum 2 years clinical nursing experience
• Home health field experience required
• Strong hands-on OASIS experience
• Proven leadership, communication, and organizational skills

Responsibilities

• Provide clinical leadership and supervision to field nurses and therapists
• Oversee OASIS and HCHB workflows for accurate, compliant documentation
• Review and manage care plans (485s) to support quality outcomes
• Lead onboarding, training, and performance evaluations for field staff
• Manage triage calls and offer real-time clinical guidance
