💼 About This Role

You'll conduct outreach and case management for high-risk patients, linking them to community resources and clinical services. You'll advocate for patients' health and well-being by addressing social determinants like food, housing, and transportation. This role is part of an innovative program in collaboration with CHCN, serving a diverse community in Oakland.

🎯 What You'll Do

• Conduct outreach and enroll high-risk patients in the program.
• Assess patients' needs and connect them to community resources.
• Advocate for patients and help remove barriers to health.
• Interface with medical providers and document in medical records.

📋 Requirements

• Bachelor's in Social Work or related field or 3 years experience
• Fluent in Cantonese and/or Vietnamese
• Experience in community-based health care capacity
• Valid driver's license and auto insurance
