💼 About This Role

You'll conduct in-home health risk assessments for Porter's members in Flint, MI, using evidence-based screenings and point-of-care testing to guide care management.

🎯 What You'll Do

• Visit members within geographic area for in-home health risk assessments
• Evaluate past medical history, review of systems, and medication reconciliation
• Perform comprehensive physical exams and point-of-care testing
• Formulate list of current and past medical conditions for care management

📋 Requirements

• Valid Michigan NP license
• 3 years of patient care experience
• Experience in primary care, adult, or geriatric
• EMR skills
