Description

You will coordinate care for senior patients at One Medical, a primary care practice, by managing referrals, conducting pre- and post-visit check-ins, and supporting clinic operations across multiple Oakland-area locations.

Requirements

• 1+ year in a high-touch patient-facing healthcare role
• Strong EMR system proficiency
• Proficient in Google Workspace
• Excellent written and verbal communication skills
• Self-motivated with ability to work independently and adapt to change

Responsibilities

• Manage care coordination including specialist referrals and appointment scheduling
• Conduct pre-visit screenings and post-visit check-ins for high-risk patients
• Support preventive care through panel monitoring and proactive outreach
• Provide patient education on chronic disease management using motivational interviewing
• Contribute to daily clinic operations including phone support and front desk assistance
