Description

Lead a hospice interdisciplinary team to deliver high-quality end-of-life care, develop community relationships, and recruit team members. This role combines clinical guidance with leadership development.

Requirements

• Current RN license in VT and NH
• Former hospice experience required
• Desire to lead an interdisciplinary care team
• BSN or Masters preferred
• Management experience strongly preferred

Responsibilities

• Support team with clinical guidance for patient and family needs
• Participate in Performance Improvement Quality Assurance activities
• Develop and mentor future hospice leaders
